Please ensure the booking name is the person attending on the day, if you are the parent/guardian please read the term and conditions before checking the box.

Also please put your son or daughters name attending, and your contact details for emergency ie email and phone.

if you are over the age of 18 then please just put your details in as follows.

• Smoothstar Day
  if you have booked the special events Smoothstar day it will be at Palm Beach netball courts at Melaleuca Drive.
• Surf Coaching
  Our usual lessons for week to week coaching will be at Burleigh Heads in front of the nook cafe under the burleigh Pavillion.
• School Holiday Program
  Will provide a pick up and drop off service. Please ensure you fill out your address when making your booking.
